If you are an outside organization: (e.g., Boy Scouts, Soccer Leagues, or non-district individual(s)) you will need to submit a Certificate of Liability Insurance as well as a signed Hold Harmless and Indemnification Agreement rev. 2-1-13 in order for the Facilities department to process and approve your request. This requirement will not be waived under any circumstance. These forms must be submitted with your online request.
If athletic fields and parking lot facilities are to be requested you will also need to submit the appropriate form(s) below:
All flyers which are intended to be distributed at school sites must first be reviewed and approved by the Assistant Superintendent of Educational Services. Please read the procedures for flyer distribution to PVUSD sites and students below.
