Certificated Evaluation System
Forms and Questions

The Evaluation Tracking System Database for
Teachers and Administrators
This customized database solution was designed to make the teacher evaluation process more streamlined and convenient for PVUSD teachers and administrators. This program is very easy to use and will allow staff to manage all of their evaluation forms in one place. It also creates uniform, typed forms that can be printed and filed.

Windows (PC Users) Versions

We made some improvements to the database and you can now find Version 2.5 posted below. If you have downloaded a previous version of the database you can simply import your data into this new version using the "Import from Prior Version" Button on the Front Page.

Administrator Version for Windows

Downloading Instructions for PC (Windows) Users

Step 1: To download one of these database files, click on the link for the version you need: PC users should click on either the “Teachers' Version for Windows” or the “Administrators' Version for Windows” link.

Step 2:
PC users should save the file (pvusdTEV.exe or pvusdAEV.exe) on their desktop in a folder where you will be able to find it easily.
