Relationship between the Evaluation Plan and Results

for Continued Program Eligibility

Our Evaluator, Karl Forest will be responsible for providing state reports as required. Data gathered in all phases will be sent to and monitored by Mr. Forest. It will be his responsibility to collect, prepare and present up-to-date data to teachers, administrators and CDE representatives at any time. Using this data, he will write required reports in a way that allows the CDE to monitor program success. The role of our Evaluator is explained in context to activities on the following pages.
